What are Premiums if you are paid cash?

Hi,
I am teaching privates for the most part and get paid cash as a result. Will the National Health Plan make me pay a monthly premium during my second year based on what they THINK I earned? Or are we all supposed to follow the honor system and state our actual earnings?

Also, would I just estimate my annual income for tax purposes?
